I think the CST is a good idea to pursue for the hypertonia. Here's some information on clamping with information on oversupply included:

I have had good success with tongue thrusting with this technique. Make sure the baby is latched really well, here's a link on that:

and then when the baby sucks apply moderate pressure with one finger to stroke from the baby's chin to the adam's apple. I have found this "reminds" the baby to suck correctly. Be sure you stroke in the same rhythm as the baby is sucking. You can do the stroking with the index finger of the hand that is supporting the breast. It's really important that you stroke front to back ONLY. Also, watch for the baby's response, see if it feels better on the nipple and see if the baby is tolerating it ok. If it isn't working as in makes your nipple hurt more or the baby resists it stop doing it. This is my version of suck training at the breast.

When I do suck training, which is very very rarely, I teach the parents to do it only for a few sucks just prior to bf, not for prolonged periods.

Make sure you are not touching any part of the baby's head above the ears when you are trying to latch. Many babies will arch and even clamp when the back of their head is touched. Some babies are so sensitive to this that they even do it if a pillow or blanket is touching them.

If you have yeast you need to treat both mom and baby at the same time even if the baby has no symptoms. I don't understand why some doctors don't understand that cross contamination is a big issue. Here's more on yeast:

Being fair doesn't make you more vulnerable to nipple soreness. Nipple soreness is an indication of a problem and NOT a normal expected part of bf. Be persistant in getting the help you need. If your LC is not being effective in helping you to improve I'm sure she would welcome a second opinion.

It looks as if I have a classic case of oversupply and strong let down. Given my sore nipples I did fall into the trap of wearing breast shells way too much which led to (1) increased supply and leaking and (2) perhaps thrush due to moist environment. At any rate, I have pretty much stopped using them in favor of breast pads. I believe that the dryer pads plus the vinegar and Monostat have begun to help.

However, I still really need to meet with the LC next week b/c there is still the clamping. It may very well be in response to oversupply b/c today Madeleine seemed to be clamping more - most likely because I was not leaking excess milk into the shells before feeding her so letdown was stronger than usual. In fact, she was in the habit of pulling off w/o letting go today (OUCH!) You'll be glad to know that I switched to the side-lying position and she settled right down for a good long feeding. Much better!

My game plan is:

- meet with LC next Monday to reevaluate thrush and hypertonia plus address latch, etc.

- see pediatrician Thursday to obtain prescription for Maddie's thrush if necessary (it's her 2 month appt)

- visit CST following Tuesday if necessary

I'll definitely let you know how each of these visits go. Thank-you so much for your help and all of the information. I want to make this work and the more I know the better!

2ND REPOST for southernlove re: Question about plugged duct, yeast... (M)

I'm breastfeeding my 8 month old DD, and I'm having SEVERE pain in my right nipple while nursing. I had a plugged duct in my right breast two days ago that feels like it has cleared up, but the all day long I've had the worst, most agonizing pain in the nipple on that side. The pain lasts the entire time that DD is latched on. There is a very small white circular spot on the nipple that showed up shortly after I noticed the plug. It is not raised at all. This was my third plugged duct (the others were in the same place as this one). With the other plugs I got the same white spot, but the other two times, the white spot disappeared when the clogged duct resolved. This time, the spot is still there. I've also had a few shooting pains throughout the day in my right breast while not nursing. While nursing, the entire breast kind of feels like it's burning during letdown, then there's just the constant nipple pain. DD has had an on again/off again rash for the past two weeks. DD's hands go straight to it during diaper change, so I think it must be itchy. Balmex doesn't seem to have much of an effect. I called her pediatrician (ended up speaking with the nurse) last week about it because it didn't look like anything my three boys have had. It is just in the front, looks like red dots, and is not in any of the folds of her skin at all. The nurse suggested cutting down on DD's juice consumption. I told her that DD does not drink ANY juice - is strictly breastfed - and doesn't really even eat solids. I asked about fungal infections, and the nurse said that that would be mainly on her bottom - not in the front. Anyway, for the past two weeks, sometimes the rash is there, sometimes it's not. It's very strange, and seems to be related to her poops. When we change her diaper in the morning, it's full of urine (probably about 20 pounds worth), and there's no sign of rash. Anytime she poops, the rash is there. Because she is crawling and spends most of her time on her stomach, whenever she poops, it end up in the front of her diaper. I told the nurse this, but it didn't seem to change her answer to me of cut out juice, use bag balm (can't find it and don't think it would do any better than Balmex), soak her in baking soda dissolved in warm water in the tub(which I haven't done because I don't think it would help her problem). Well, now with the pain I'm having, I'm wondering if there's yeast involved. I've never had a yeast infection or thrush before, so I don't know what I'm really supposed to be looking for. I can take DD to pediatrician (not that I think much of that office), but what should I do? We just moved to another state from where DD was born, so I don't have a doctor here. I wouldn't even know what kind of doctor to see. Should I try to track down a lactation consultant?

I'm not sure what to do. Could this plugged duct be related to yeast? I breastfed my youngest son for a year and never had any problems whatsoever. Any help would be appreciated. I'm really not looking forward to tonight. DD usually wakes up to nurse 3 or 4 times a night. I'm in for a very painful night....

I'm working through all of the information and trying to figure out what my "nipple problem" is.

Thanks to those who gave advice. I'm still hanging in there!
