I loved An Echo in the Bone by Diana Gabaldon; it's book # 7 in the Outlander series. (She also writes the Lord John books, of which there are three now, I read these too. Some who don't were confused about a character this book who was also in Lord John and the Brotherhood of the Blade, on an Amazon list.)
The Outlander series is about 20th Century Claire, who right after WWII, in which she was a battlefield nurse, she goes with her husband Frank on a second honeymoon in Inverness, Scotland. They go to some standing stones, Claire hears a humming, and then she's in ~1742 in among a skirmish between English soldiers & some Scottish cattle rustlers. There she meets Jamie Fraser and many other Highland warriors.
Book 7 begins in 1776, in Fraser's Ridge in the mountains beyond Wilmington, NC. Claire became a surgeon and attempts to do in 1777, what she was doing in 1969, by making her own ether, penicillen and other drugs. Jamie tries to stay out of the way of his illeligimate son William, an English Lord and officer, not very successfully. Jamie and Claire have "been together" for 35 years and this is also about their families and friends and the wars they fight.
It is long and complex historical fiction and it's also a lot of fun!
