1- Do you go to the dentist regularly? yes!

2- Assuming you have insurance, do you use a dentist that's "in network" or do you go to one that YOU like & just eat the extra cost? mine is in the network. i've also had procedures that haven't been covered by the network but i didn't know that until i received the bill.

3- Ever had a complete physical - not because there was something wrong, but just to have a physical? yep! i have had one for the last 2 years.

4- How's your eyesight? (Glasses? Contacts?) 20/20 vision- no contacts or glasses

2- Assuming you have insurance, do you use a dentist that's "in network" or do you go to one that YOU like & just eat the extra cost? My local dentist is not in network, but his fees are low and it never costs me anything.

3- Ever had a complete physical - not because there was something wrong, but just to have a physical? Yes, but in stages. I stay current on mammograms and colonoscopies, have blood tests every 6 months, but only have EKGs before surgery or a procedure.

4- How's your eyesight? (Glasses? Contacts?) Oh boy. I'm not the cl- on the Visual Impairments board for nothing!
