It has been a while since I've done the research on the glucose screening test, and I'm wondering if a multiple gestation would skew the outcome of the test? With my last pregnancy I did not want it (though I was talked into it) after I read about it and the false positives. Anyway, I got to thinking that I am probably going to be coerced into it again soon and was wondering whether or not the results are going to provide any meaningful (as meaningful as the test can be) data for me. I know my doctor didn't advise for the triple screen because it's not a reliable test when there's more than one baby--would it be the same with the glucose screen?

Congrats!  Twins, huh?

I don't know if that has ever really been researched...but if anyone knows, Henci will.  I hope she has the time to answer!

I'll comment though on the feeling of being "coerced" into the test...sometimes that can mean that it is time to find a new care provider...but honestly, you are going to be very hard put to find one that doesn't insist on the GD screen--even my CPM did.  I chose to just not make a battle about it...but also not do anything that might cause me to fail the test.  For example...in my second pregnancy I let my daughter drink some of the glucola.  In my 4th, when the lab was slow about drawing my blood I answered "yes" when they said "has it been about one hour since you ate?" (my midwife uses a food based test) even though it had been 1.5 hrs--that's "about" an hour, right? ;-)

Thanks for your comments. I am pretty sure I'll have to take it one way or another. Grr! I didn't have to do it with my first (also twins), but they rarely do any mass screening like that (and group b strep) in the country I gave birth in. If I go with a midwife, perhaps she'll agree to a food test, too.

I recommend raising this point with your dr. I would certainly think that a multiple pregnancy would affect glucose values. The physiologic reason why glucose values are normally elevated in pregnancy is to fuel the growth of the baby, and you've got two of them. If your dr plans to use the same values, I would insist on seeing research that confirms that it isn't necessary to adjust them because normal ranges do not differ.

Are you sure that you don't want to "just say no"? You do--or at least you should--have the right to informed refusal. There are some pretty respectable people saying that routine testing should be abandoned because it doesn't improve outcomes, among them the authors of the evidence-based A Guide to Effective Care in Pregnancy and Childbirth.

Thank you for taking time to answer my question. I will try with everything in me to refuse this test. I just got to thinking about what my previous doctor had said about twin gestations skewing numbers for the triple screen, and it would make sense that this would be true of any other test/screen. I'm still looking for the perfect doctor (while trying to decide on a homebirth, as well) who will be supportive of a twin VBAC. My 1st birth, the c-section, was with twins, too. I've got lots of fighting to do, and not just with the GTT!
