Hi all -

 

I haven't been here in awhile, but have stopped in to get some good advice in the past (thank you!)

I am generally pro-vaccine and had the opportunity to get my 4 year old the H1N1 today -- I jumped at it since it is scarce around here and I have a 4 month old daughter.  They gave him the flu mist.  No problem -- he is healthy with no immune issues.  Then my sister told me he could give the flu to my daughter because it's a live virus.  My ped KNOWS I have an infant and didn't mention this.  How worried should I be?  And for how long?  48 hours?  I asked my four year old to not kiss or touch the baby right now and he's trying.

I normally don't panic about this kind of stuff, but two years ago I lost my second child when he was 10 days old to respiratory failure most likely caused by a viral infection.  So I'm terrified of my baby girl getting sick

Any words of comfort or advice??

Can people receiving the nasal-spray flu vaccine LAIV pass the vaccine viruses to others?

In clinical studies, transmission of vaccine viruses to close contacts occurred only rarely. The current estimated risk of getting infected with vaccine virus after close contact with a person vaccinated with the nasal-spray flu vaccine is low (0.6%-2.4%). Because the viruses are weakened, infection is unlikely to result in influenza illness symptoms since the vaccine viruses have not been shown change into typical or naturally occurring influenza viruses.

Can contacts of people with weakened immune systems get the nasal-spray flu vaccine?

People who are in contact with others with severely weakened immune systems when they are being cared for in a protective environment (for example, people with hematopoietic stem cell transplants), should not get the nasal spray vaccine, including the 2009 H1N1 nasal spray vaccine if they will come into contact with the severely immunocompromised person within 7 days of vaccination. People who have contact with others with lesser degrees of immunosuppression (for example, people with diabetes, people with asthma taking corticosteroids, or people infected with HIV) can get the nasal spray vaccine.

Hi Gina - there are risks due to viral shedding from the nasal H1N1 vaccine but those risks are so small that I think the risks associated with a young infant contracting H1N1 infection from an infected sibling is greater. IOW, I'd be more worried about your 4 month old with unvaccinated siblings in the household than I would with siblings who received the nasal H1N1 vaccine. You did do the right thing and the nasal H1N1 vaccine is the better choice compared to the shot, imo, in terms of immunity.

Thank you both so much!  I'm feeling calmer about it today.  Eve, that was exactly my thought today:  Grace's risk  of getting sick is much greater if Noah contracted H1N1 then her risk of getting sick from his vaccine.  And it's definitely becoming widespread at his school right now.

I didn't know that the intranasal vax offered better immunity than the shot, but that makes sense.  I got the shot today from my OB.

After reading Anne's post though, I will postpone our visit to a friend who started chemo this week.  How long does the virus "shed"??

In the package insert for the seasonal flu mist there is a reference to a study which documented shedding at 21 days.  There was another study that suggested that it is ok to be around immune deficient individuals after 7 days.  Here are links:
